This Notice also does not apply to the processing of your Personal Information during your participation in the Trial. If you become a participant in the Trial, please refer to the informed consent form you will sign for more information about our collection and processing of your Personal Information.
Personal Information we collect on the Website and Application includes:
- First and Last Name;
- Email Address;
- Zip Code;
- Health and/or genetic information; and
- Any other information you provide through webforms on our Website or Applications.
We also collect non-personal information about your visit to our Website or use of our Applications. For example, we may capture your browser type, settings, device information, and IP address in order to monitor the number of people that visit our Website or use our Applications and compile aggregated statistics to improve our Website and Applications. Our main purposes for collecting information about you are to provide you with information about our Trial, to assess your (or your loved one’s) compatibility with the Trial, and to begin enrollment in the Trial.

Lantern may disclose the Personal Information we collect through the Website and Applications to Trial sites (e.g., the doctor’s office, clinic, hospital, or other healthcare facility where the Trial is conducted) and the clinical research organization (“CRO”) assisting us with the Trial.
Additionally, Lantern may disclose the information we collect about you, including Personal Information, to our service providers, such as our web or application developer, so that they may provide services to us; as required by law or law enforcement; or in the event of a merger, sale, or acquisition. Lantern does not sell Personal Information collected through our Websites or Applications.
